Forms for Sound Body Sound Mind DPS Fitness Centers

Denver Public Schools' Office of Risk Management has allowed the use of the Health Appraisal and Medical History Questionnaire to determine if potential members need medical clearance from medical providers before participating in exercise in SBSM Fitness Centers. Medical clearances are still highly recommended but not required unless the Health Appraisal and Medical History Questionnaire determines a need for medical providers' clearance.

• Pre-Participation Screening Questionnaire (English)/Cuestionario previo a la participacion (Spanish)—Denver Public Schools' Office of Risk Management has determined the use of Pre-Participation Screening Questionnaires to determine if potential members need medical clearance from a medical provider before participating in exercise in SBSM Fitness Centers. Medical Clearance Forms are still highly recommended but not required unless the Per-Participation Screening Questionnaire determines the need for Medical Clearance Forms.
