Check-Out/Check-In and Policy
Please note the following guidelines. If we all abide by these guidelines, everyone benefits.
- To check out equipment, download and complete an Equipment Request (PDF or Word). Fax this form to 720-423-8100, email it to christa_taylor@dpsk12.org, or send it to the Physical Education and Dance Resource Center,
1330 Fox Street, Third Floor East. Telephone requests will not be accepted.
- Only physical education and dance teachers are allowed to check out materials and only for school or office use. No personal use is allowed.
- When completing the equipment request form, enter months in sequence
(for example, January, February, March).
- If you want only part of an order (for example, bowling equipment, no carpets),
state it on the request form.
- Materials are loaned for two weeks at a time. Please plan accordingly.
- Denver schools are divided into two delivery days.
- Equipment will be delivered or picked up by 7:00 a.m. on designated delivery or pickup date.
- Prepare items the day before the 7:00 a.m. pickup in the same order as received.
- Contact Christa Taylor if part of an order is missing or damaged upon arrival.
- Contact Christa Taylor if you do not receive equipment on the day requested.
- Contact Christa Taylor ahead of delivery date if you find you will not use items requested.
- Continued failure to have materials ready for pickup will be cause for denial of future use of equipment.
- Broken, lost, or stolen items will be recorded, and a replacement fee will be charged to your school.
- Stilts are to be used indoors only!
